Setting: Skilled Nursing Facilities (Multi‑Site)

Rehab Model: In‑House Therapy + Contract Rehab Programs

Seeking a Corporate Director of Rehabilitation to lead and elevate rehabilitation services across a portfolio of Skilled Nursing Facilities throughout southern CA. This is a strategic, high‑impact leadership role responsible for operational oversight, clinical quality, staffing performance, and program development across both in‑house therapy teams and contract rehabilitation partners.

Key Responsibilities Provide executive‑level leadership for multi‑site SNF rehabilitation operations, ensuring consistency, compliance, and high‑quality clinical outcomes. Oversee both internal therapy departments and external contract rehab providers, optimizing performance, communication, and alignment with organizational goals. Monitor KPIs, productivity, staffing models, and financial performance across all assigned facilities. Lead, mentor, and support therapy leaders and clinicians across multiple sites, fostering a culture of accountability, engagement, and professional development. Evaluate and enhance therapy programming, including specialty services, clinical pathways, and evidence‑based practice initiatives.

Qualifications Licensed PT, OT, or SLP (required).

Minimum 5 years of progressive leadership experience in post‑acute rehabilitation, with multi‑site oversight preferred.
